Sanford Health is seeking a dedicated RN Transplant Coordinator to join our Healthcare & Medical Services team and play a central role in delivering safe, compassionate, and highly coordinated care to transplant candidates, recipients, and living donors. As an integral member of a multidisciplinary transplant team, you will combine advanced clinical expertise with exceptional communication and organizational skills to guide patients and families through every stage of the transplant journey. In this role, you will conduct comprehensive nursing assessments, participate in the evaluation of potential transplant recipients and donors, and coordinate the full continuum of care from initial referral to long-term follow-up. You will serve as a primary point of contact for patients, their families, referring providers, and internal care teams, ensuring timely information sharing, seamless transitions of care, and a consistently supportive experience. The RN Transplant Coordinator develops, implements, and regularly reviews individualized care plans, closely monitoring patients’ clinical status, lab results, and diagnostics while escalating concerns to transplant physicians and advanced practice providers as appropriate. You will provide detailed pre- and post-transplant education, helping patients understand complex procedures, medication regimens, lifestyle changes, and ongoing follow-up requirements, while offering emotional support and advocacy throughout the process. Working within Sanford Health’s supportive and inclusive culture, you will collaborate closely with social workers, pharmacists, dietitians, financial counselors, and other specialists to address psychosocial, financial, and educational needs. You will document all care and communication in the electronic health record and maintain accurate transplant program records and registries, supporting regulatory and accreditation requirements aligned with UNOS, CMS, and internal quality standards. The RN Transplant Coordinator also contributes to quality improvement initiatives and outcomes tracking to enhance program performance and patient safety. You may participate in on-call or extended hours coverage to support urgent transplant-related care and coordination, helping ensure that patients receive timely, high-quality services.
