Maplewood Brewster LLC, a senior living community providing assisted living and memory care services in Brewster, MA, is seeking a dedicated RN/LPN Wellness Nurse to join our Healthcare & Medical Services team. In this role, you will deliver resident-centered clinical care that supports the health, safety, and overall well-being of older adults in a warm, engaging, and supportive environment. Working closely with residents, families, physicians, and fellow team members, you will help create and carry out individualized health care plans that enhance quality of life and promote dignity, independence, and comfort. As an RN/LPN Wellness Nurse, you will complete comprehensive nursing assessments, monitor changes in residents’ conditions, and ensure that care plans are accurately implemented and updated. You will administer medications and treatments according to physician orders and community policies, provide wound care and chronic disease management, and respond promptly and effectively to urgent or emergent resident needs. In partnership with care staff, you will model best practices in infection control, fall prevention, and safety, while maintaining accurate, timely documentation in the electronic medical record. You will play a key role in coordinating care with physicians, therapists, and other health professionals, communicating clearly with families about resident status and care needs, and supporting smooth transitions of care such as hospital admissions or returns. You will provide guidance and clinical oversight to care associates, offering coaching, mentoring, and on-the-job training to support a high standard of practice and a culture of continuous learning. Collaboration and teamwork are central to this position, as you work side by side with colleagues to create a positive, respectful, and compassionate environment for residents and staff alike. In this on-site wellness-focused nursing position, you will also help plan and support wellness initiatives that encourage mobility, engagement, cognitive stimulation, and overall health. You will advocate for residents’ preferences, help them and their families understand care options, and contribute to care conferences and interdisciplinary meetings. Consistent compliance with state regulations, company policies, and professional nursing standards is expected, along with participation in quality improvement efforts within the community.
