Maplewood Brewster LLC is seeking a dedicated RN/LPN Wellness Nurse to join our Healthcare & Medical Services team in our assisted living and memory care community in Brewster, MA. As a Wellness Nurse, you will play a key role in supporting resident-centered care, ensuring that each individual’s physical, emotional, and cognitive needs are met in a safe, respectful, and compassionate environment. In this role, you will be responsible for providing direct nursing care to residents, including comprehensive health assessments, medication administration, wound care, and monitoring of chronic conditions. You will develop, implement, and update individualized wellness and care plans in collaboration with physicians, families, and the interdisciplinary care team. As a key clinical resource on the floor, you will oversee and support care staff, ensuring that all care is delivered according to residents’ service plans, current standards of nursing practice, and regulatory requirements. The RN/LPN Wellness Nurse will closely monitor changes in residents’ conditions, respond promptly to health concerns, and coordinate appropriate interventions, including arranging follow-up appointments, communicating with primary care providers and specialists, and facilitating transitions of care as needed. You will document all care and observations accurately in the electronic medical record, maintain medication and treatment records, and help ensure compliance with state and federal regulations, including infection control and safety protocols. Working in our memory care and assisted living neighborhoods, you will use specialized approaches to support residents living with Alzheimer’s disease and other forms of dementia. This includes using validation, redirection, and other evidence-based techniques to promote comfort, engagement, and dignity. You will work closely with activity and life enrichment teams to encourage participation in wellness programs, exercise, and therapeutic activities that support residents’ overall well-being.
