Portsmouth Regional Hospital is seeking a compassionate and detail-oriented Registered Nurse Inpatient Wound Care professional to join our Healthcare & Medical Services team. In this role, you will provide specialized wound care to adult and geriatric inpatients across multiple units, collaborating with physicians, advanced practice providers, and the interprofessional care team to optimize healing and prevent complications. You will perform comprehensive skin and wound assessments, develop and implement individualized wound care plans, select and apply appropriate dressings and advanced therapies, and closely monitor patient progress, documenting all care in the electronic medical record with accuracy and timeliness. Working in a fast-paced, innovative environment, you will serve as a clinical resource and educator for bedside nurses and support staff on evidence-based wound care practices, pressure injury prevention, and product selection, while assisting with hospital-wide initiatives to reduce hospital-acquired pressure injuries and improve skin integrity outcomes. You will coordinate care transitions and provide patient and family education on wound management, infection prevention, and follow-up care to support safe discharge planning and continuity of care. The role requires strong clinical judgment, excellent communication and teaching skills, and the ability to collaborate effectively with case management, rehabilitation, nutrition, and surgery to support comprehensive patient care.
