Life Care Centers is seeking an experienced and detail-oriented MDS Coordinator to support resident-centered care and reimbursement processes in our Healthcare & Medical Services department. In this role, you will lead and coordinate the Minimum Data Set (MDS) assessment and care-planning process for residents in our skilled nursing and rehabilitation community. Working in a patient-first, family-focused environment, you will collaborate closely with nursing, therapy, social services, and medical providers to ensure accurate, timely, and compliant assessments that reflect each resident’s clinical status, goals, and preferences. As the MDS Coordinator, you will be responsible for initiating, completing, and submitting MDS 3.0 assessments in accordance with federal and state regulations, payer requirements, and Life Care Centers’ standards. You will review clinical documentation, interview residents and families when appropriate, and work with the interdisciplinary team to develop, update, and monitor individualized care plans that promote optimal functional outcomes and quality of life. You will serve as a resource on the Resident Assessment Instrument (RAI) process, ensuring that staff understand documentation requirements and how their charting supports accurate coding and reimbursement. This position requires strong clinical judgment, excellent organizational skills, and a thorough understanding of Medicare and Medicaid guidelines related to skilled nursing reimbursement. You will analyze assessment data to support appropriate case-mix, assist with monitoring quality measures, and contribute to quality assurance and performance improvement (QAPI) initiatives. In partnership with facility leadership, you will help identify trends, address variances, and support best practices that align with Life Care Centers’ commitment to compassionate, high-quality care. The MDS Coordinator role is best suited for an RN with long-term care or post-acute experience who is comfortable managing multiple deadlines, coordinating across disciplines, and communicating clearly with residents, families, and regulatory agencies as needed. You will work on-site within the facility, participating in care conferences, team meetings, and routine chart reviews to ensure the MDS and care plan accurately capture resident needs.
