About the Role:

Responsibilities:

• Conduct thorough assessments and develop individualized care plans for wound care patients.
• Administer wound care treatments and monitor patient progress effectively.
• Educate patients and families on wound care management and health maintenance.
• Collaborate with healthcare teams to ensure comprehensive patient care.
• Document patient care activities accurately and timely in electronic health records.
• Provide compassionate support and emotional care to patients and their families.
• Stay updated on best practices and advancements in wound care management.
• Participate in ongoing training and professional development opportunities.

Requirements:

• Current LVN license in California with Wound Care Certification.
• Minimum of 1-2 years of experience in home health or wound care settings.
• Strong assessment and critical thinking skills.
• Excellent communication and interpersonal abilities.
• Compassionate demeanor with a patient-centered focus.
• Ability to work independently and as part of a team.
• Proficient in electronic health record systems.
• Current CPR certification and knowledge of infection control practices.
